Hi Bob, Yes, Safety is the concern for me. It is just my opinion, but I think that neck knives should be as light as possible and have the best possible sheath retention. Having an opened knife loose under my shirt is a scary situation. We don't make very many neck knives. My Swick design is offered...

Edit; I lied, In speaking with R&D, it seems like there was one other case similar. Might be contamination by the serration wheel? It's being investigated.
